How to donate Data?
We are actively asking patients to donate their data after a clinical examination has taken place and the diagnosis has been confirmed. We use special envelopes in which both the data and a corresponding declaration of consent can be sent to our offices. The envelopes are provided with QR-Codes so that the patient can always access or delete the data if necessary.

Then download the Informed Consent Form (English Version) or the Teilnehmeraufklärung (German Version), fill out the last two pages and mail your digital images (in a reinforced envelope) to:
